About usThe Vale of Glamorgan Long Term Care Service assesses and supports people and their carers with long term needs for care, and support. The team responds to individuals whose needs may be complex or require on-going monitoring and support in order to achieve their wellbeing outcomes. The team supports people where short-term services are not appropriate.The service also incorporates a dedicated Review team where this post is located.This team completes scheduled and unscheduled reviews to evaluate progress towards achieving outcomes, establishing that any managed care and support arrangements remain eligible for local authority support, and completing re-assessments where required.Experience of working with adults is desirable but not essential.
