About usThe Vale of Glamorgan Long Term Care Service assesses and supports people and their carers with long term needs for care, and support. The team responds to individuals whose needs may be complex or require on-going monitoring and support in order to achieve their wellbeing outcomes. About usThe successful candidate would have the opportunity of working within Adult Services- Vale Community Resource Service (VCRS) You would support the social work function of the multidisciplinary integrated health and social care reablement team.VCRS is an integrated Health, Social Care and Third Sector Team based at Barry Hospital.VCRS work with people in their own home to maximise functional independence in activities of daily living (ADL), thus reducing the need for admission into hospital and longer-term care and support services. It also offers therapeutic intervention and reablement support to people following an admission into hospital, so that they can return home as timely as possible.
